At the beginning of EarlyTMS use, contents in ticket template are empty. Follow these steps to create sample ticket template.

1. Click administration
2. Ticket Template Management
3. Click the ticket template created in section 2
4. Click 'Add new field'
5. Change the name of field to First Name in 'Name' under Properties
6. Click 'Add new field'
7. Change the name of field to Last Name in 'Name' under Properties
8. Click 'Add new field'
9. Change the name of field to Address in 'Name' under Properties
10. Click 'Add new field'
11. Change the name of field to City in 'Name' under Properties
12. Click 'Add new field'
13. Change the name of field to State in 'Name' under Properties
14. Click 'Add new field'
15. Change the name of field to Zip Code in 'Name' under Properties
16. Click 'Add new field'
17. Change the name of field to Phone Number in 'Name' under Properties
18. Click Input Type and select phone no.
19. Click 'Add new field'
20. Change the name of field to email address in 'Name' under Properties
21. Click Input Type and select email
22. Click 'Add New Group'
23. Change the name of Group to ticket Information in 'Name' under Properties
24. Change the columns in a row to 3 under Properties
25. Change the name of field to status in 'Name' under Properties
26. Click Input Type and select [Status]
27. Click submit
